Service Coordinators, Foster Providers, Residential and Vocational Staff interested in becoming able to assist individuals with intellectual and developmental disabilities to obtain a Health Care Representative will need this class. Successfully completing this course provides the training necessary to appoint a team-named Health Care Representative for those who are unable to appoint their own Health Care Representative. Others who are interested in learning more about Health Care Representatives are also welcome to attend as space allows.

Cost

$10/Co-op member, $20/non-member. (Payable to 'CCSS.')
